Being Discharged

Before you can leave the hospital your physician will place an order for your discharge. The staff will assist you as needed with post-hospital care plans and this will be outlined for you in the discharge instructions.

When you have been officially discharged a hospital volunteer or staff person may take you out of the hospital in a wheelchair, to assure your comfort and safety on the way to the car. Your family or caretaker can help you from there.
